Please fill out our Travel Risk Assessment Form for your trip. To allow sufficient time for the nurses to complete their assessments and order in any vaccinations please submit the form at least 6 weeks prior to your departure date.
Unfortunately, we are not able to accept forms if the departure date is less than 2 weeks away, and we would advise that you urgently contact a private travel clinic who will be able to assess you and administer any vaccinations required.
If you know that you will require vaccination against Yellow Fever please make arrangements with a registered Yellow Fever vaccination centre as we are not able to administer this vaccination here.
